OneCareVt nurse consultant speaks for community health initiative

Tuesday, Feb. 21, 4 p.m.—WOODSTOCK—There are many organizations in communities working together to improve health and wellness. On Tuesday, Feb. 21, 4 p.m., Sandy Soho will give a presentation “Live Long and Prosper” at the Norman Williams Public Library, and invites community members at large to participate in spreading the word about healthy behaviors and to celebrate what is already happening. She will discuss hopes and dreams for promoting healthy living going forward, as well.
Soho is a Nurse Clinical Consultant for OneCareVT, an accountable care organization in Vermont. In her role with OneCare, she is on the steering committee for the Windsor Health Service Area Accountable Community for Health. The 3-4-50 Project is the first initiative being promoted by this group.
